Case Study: Population Health Management
A major regional healthcare system sought to better utilize population health data to formulate care plans and more effectively track and engage with patients across the care continuum in order to improve outcomes for patients with complex healthcare needs.
Huron collaborated with the client to advance population health efforts by assessing and improving outreach and care coordination workflows, aggregating population management data, and optimizing a patient portal. The project teams worked to establish a strong process for developing patient- and family-friendly care plans that foster direct patient interaction and meet requirements for Accountable Care Organizations (ACO), Patient-Centered Medical Homes (PCMH), and The Joint Commission (TJC).
